I filled out the paperwork and mailed in. When they emailed me the info some dates were wrong, due to my printing. Excellent customer phone support, they fixed it right away and emailed the corrected info while on the phone. Also answered some other questions I had. Would buy again. Lab fee $129.00, I fiqure about average
